Not sure if that is something that was homemade or purchased as I don't see any other listings that have it. Cover has been washed and has one small stain in one corner and one small seam separation on the side (all pictured). Tested and unit seems to run properly. Does not have the velcro straps, but my understanding is those can be purchased separately easily and in many places. Heating pad itself looks to be in very good condition.
