Louisville. 401 South 2nd Louisville, Kentucky (Phone)" inset on front and a matching 7 1/2" wide by 4" tall unused mailing envelope with the same logo & info- In very fine condition with only light wear consistent with age and normal use- A Real VINTAGE set ready for display!
